For Patients & Families

Q: Who provides care during visits?

A: Care is led by Amber N. Casarez, AGNP-C, a board-certified Nurse Practitioner with training in advanced wound care. You’ll always be seen by a licensed provider.

Q: Do you accept Medicare?

A: Yes. Healytics is actively enrolled with Medicare and provides services that include wound evaluations and CMS-covered biologic grafts, when medically necessary.

Q: Where do visits take place?

A: We offer care directly at home, bedside, or within assisted living or hospice settings for eligible patients.

Q: What kinds of wounds do you treat?

A: We treat diabetic foot ulcers, pressure injuries, post-surgical wounds, arterial and venous ulcers, traumatic wounds, and skin tears—among others.

Q: Will I receive follow-up care?

A: Yes. We return weekly or as medically indicated to assess wound progression, reapply grafts, and update your care plan.

For Referring Providers & Facilities

Q: What services does Healytics provide?

A: We deliver advanced mobile wound care including comprehensive evaluations, biologic graft applications, dressing plans, and secure documentation—often within 48–72 hours of referral.

Q: How is documentation handled?

A: Each visit includes wound measurements, photography, cm² documentation, and a full chart note. We share summaries with your team to maintain care continuity.

Q: Do you work with home health or facility nursing staff?

A: Absolutely. We coordinate care with your existing teams, provide dressing plans, and offer wound education when needed.

Q: Do you take over care from primary care or wound clinic providers?

A: No. We collaborate as part of the care team and keep all referring providers updated on progress. We are an extension, not a replacement.

For Home Health & Hospice Agencies

Q: Can we refer directly to Healytics?

A: Yes. You may refer patients using our secure form or by fax. We’ll contact the patient or POA to coordinate consent and scheduling.

Q: How do you document wound status for hospice IDT or home health audits?

A: Every visit includes wound photos, measurements, dressing plan, and graft tracking. Documentation is HIPAA-secure and audit-ready.

Q: How do you avoid duplicating services with nursing staff?

A: Healytics focuses on advanced wound care and biologic procedures. Routine dressing changes or home nursing visits continue as usual, while we provide orders, oversight and specialty intervention.

Q: Will your services impact hospice billing?

A: Our services are billed directly to Medicare under our NPI and do not interfere with hospice per diem unless separately contracted.

For Billing, Compliance, and Oversight Teams

Q: Are Healytics services CMS-compliant?

A: Yes. We follow Medicare guidelines for biologic graft billing, including use of 15271–15278 CPT codes, cm² tracking, consent, and photo documentation.

Q: Is your Nurse Practitioner credentialed and authorized to bill?

A: Yes. Amber N. Casarez, AGNP-C is PECOS-enrolled with both Type 1 (individual) and Type 2 (Healytics) NPIs and practices under a collaborating physician agreement per Texas law.

Q: Do you conduct internal audits?

A: Yes. Healytics performs quarterly chart reviews, photo and graft log audits, and billing reviews led by our CFO to ensure CMS and HIPAA compliance.

Q: How do you handle PHI and HIPAA compliance?

A: All data is stored in a secure EMR. Devices are encrypted and staff are trained annually on HIPAA standards. We maintain a full breach protocol and business associate agreements.

Q: How are providers compensated?

A: All compensation models are legally structured to comply with anti-kickback laws and Stark regulations. No compensation is tied to referral volume or outcomes.
